If you have received notification that the Tribe is interested in consulting on your proposed project under the provisions of the California Environmental Quality Act (CEQA) (Public Resources Code section 21080.3.1 subdivisions (b), (d) and (e)) for the mitigation of potential impacts to tribal cultural resources, you may download a Consultation Form here:
To initiate consultation with the Tribe, the applicant must file the Consultation Application within 30 days of the Tribe’s request for consultation. The form and associated fee should be mailed to:

The Tribe will notify the project’s contact person once the application has been received. Please allow 1 to 2 weeks for the Tribal Historic and Cultural Preservation officer (THCPO) to review the project. Upon review, the THCPO will provide a Tribal Sensitivity Point System (TSPS) document as well as formal comments. Once a reasonable conclusion has been reached in good faith, consultation may be considered concluded with the Tribe.
