    I have read, understand, and consent to the submission of this form. I understand the fee submitted with this form is for the initial 15-minute consultation only. The Cultural Resources Management (CRM) Division of the Fernandeño Tataviam Band of Mission Indians (FTBMI) will review the contents of this form and any applicable documentation made available by the applicant to determine the depth of consultation and FTBMI involvement required. I understand the cost required for additional consultation is structured on a case-by-case basis dependent on the depth of consultation and FTBMI involvement required for the summaries and/or inventories provided. The consultation fee structure for my specific request will be provided to me during the initial consultation.
